SC was involved in a motor vehicle accident on July 31, 2011 in Troup County, Georgia when another vehicle failed to back out properly and maintain control of the vehicle. After this collision, SC required medical attention. He was transported via ambulance to the emergency room of West Georgia Medical Center. At the Emergency Room, the emergency room physician examined him. SC presented with complaints of pain in his low back. He was diagnosed as having acute myofascial strain. He was prescribed a conservative program of physical therapy to treat his symptoms. The goal of this physical therapy was to increase the range of motion, decrease pain, and aid the muscles and ligaments in healing. After treatment, we setled the claim with Hallmark Insurance.
