Paige Tatum and Jose Pouso were involved in a motor vehicle accident on July 29, 2009 in Fulton County, Georgia. After this collision, Paige Tatum required medical attention. She was transported via ambulance to the emergency room of Atlanta Medical Center where she presented with complaints of pain in head and neck. She was diagnosed as having closed head trauma, acute neck pain, and multiple blunt injuries. Paige Tatum was given prescriptions for the sustained injuries and was also advised to seek follow-up medical care for the injuries sustained. She continued to suffer pain and sought care from New Concept Chiropractic Center. Paige continued to suffer from lower back pain, and she was diagnosed with lumbar subluxations, muscle spasms, and lumbar sprain/strain. New Concept Chiropractic Center found that Paige had reduced range of motion thereby prescribing a program of physical therapy to treat her symptoms. Paige was also seen by South Atlanta Medical & Health Rehab and was diagnosed with: cervicobrachial syndrome, cervicocranial syndrome, lumbar and thoracic sprain/strain, and numbness and tingling of left leg. Paige Tatum was given prescriptions for the sustained injuries. Again, Paige was advised to seek additional care for the injuries sustained as a result of the July accident. Seen by Center Neurology, Paige was diagnosed with post traumatic headaches, severe soft tissue injury to the neck, right cervical radiculopathy, right shoulder soft tissue injury, low back moderately severe soft tissue, and lumbar radiculopathy. Paige Tatum was advised to continue with her chiropractic treatment, and to follow up with a specialist to have MRIs taken of her cervical spine, lumbosacral spine, and right shoulder. She secured MRI’s at Fayette Health Imaging including images of her cervical spine, lumbosacral spine, and right shoulder. She was diagnosed with having multilevel degenerative disc disease of the lumbar spine and cervical spine, and abnormality of the anterior distal supraspinatus tendon. Paige continued to suffer pain from the sustained injuries. She was seen by Peachtree Orthopedic Clinic. Paige continued to suffer from bilateral upper extremity numbness and tingling, lower back, shoulder, and cervical pain. Peachtree Orthopedic Clinic diagnosed her with cervical disc herniation, cervical stenosis, possible cervical radiculopathy, right shoulder impingement, bursitis of the right shoulder, lumbar disc herniation, possible discogenic low back pain, lumbar facet pain, and numbness/tingling upper bilateral extremities. Paige Tatum was given prescriptions for the sustained injuries. Paige was on these medicines for approximately 30 days. Upon discharge, she was instructed to rest and take prescribed medications. Paige was also advised to seek follow up medical care for the injuries sustained.
Jose Pouso was also injured and sought treatment as a result of the motor vehicle accident seeking treatment from Dr. Kallins of New Concept Chiropractic Center. Jose had injured his back in the collision and was suffering from severe lower back pain. Dr. Kallins diagnosed him with lumbar and cervical subluxations, muscle spasms, cervical and lumbar sprain/strain, and radiculopathy. Dr. Kallins also diagnosed Jose with reduced range of motion in his lower back and thereby prescribed a conservative program of physical therapy to treat Jose's injuries. Jose continued to suffer pain from the sustained injuries. Additionally, due to travel difficulties he had to transfer his care from Dr. Kallins office to Dr. Brunett of South Atlanta Medical & Health Rehab. Jose had continued to suffer from head, lower back, shoulder, and neck pain. South Atlanta Medical & Health Rehabilitation diagnosed him with cervicobrachial syndrome, cervicocranial syndrome, thoracic radiculitis, somatic dysfunction of lumbar region. Jose also had a radicular injury. The physician at South Atlanta Medical & Health Rehab referred him to Dr. Joseph Saba at Center Neurology for treatment and evaluation. Jose continued to suffer from head, lower back, knee, and neck pain. Dr. Saba diagnosed him with post traumatic headaches and blurred vision, severe soft tissue injury to the neck, left cervical radiculopathy, right shoulder soft tissue injury, low back severe soft tissue injury, lumbar radiculopathy, and knee sprains. He was advised to continue with his chiropractic treatment and to follow-up with a specialist to have MRIs taken of his cervical spine and lumbar spine. At Fayette Health Imaging, Jose had MRIs taken of his cervical spine and lumbar spine. He was diagnosed with having mild degenerative disc disease of the cervical spine and minor degenerative disc disease in the cervical spine. Due to the severity of his illness, he was given a referral to Peachtree Orthopedic Clinic. At Peachtree Orthopedic, Dr. Pollydore diagnosed him with cervical disc herniation, possible cervical radiculopathy, left shoulder impingement, low back pain, left shoulder pain, and numbness/tingling upper bilateral extremities.
Once Paige and Jose were feeling better, we were able to settle the claim against State Farm Insurance.
